A high quality comprehensive clinical evaluation within 72 hours of discharge from an acute care facility is one of the best ways to ensure patient compliance and improvement with the recommended discharge plan of care and ultimately reduce re-admissions and improve patient health. As a member of STRiVE Clinical Network, a physician led accountable care organization, we work closely with a team of nurse care managers, health coordinators, clinical pharmacists and local physicians to coordinate the post-acute discharge transition from the hospital back into the hands of the primary care physician.

We understand that patients often develop urgent concerns after surgery and that these are best handled by the operating physician. Open 7 days a week and after most offices close, we are also happy to serve as a triage site for patients with post-operative concerns and work closely with physicians to help ensure patient wellbeing and provide reassurance and support to help coordinate the patient back to their physician.
